A Practical 2 Day Point-of-Care Ultrasound Course

Ultrasound has become an increasingly important investigation in the assessment and management of acutely unwell medical patients. However, access often depends on referral to other specialists, including radiologists and cardiologists. This can introduce significant delays in diagnosis and management, potentially prolonging hospital stays. In addition, the practical barriers associated with obtaining formal imaging may even result in the investigation being deferred or omitted entirely.

Point-of-care ultrasound (POCUS) addresses these limitations by placing focused ultrasound directly in the hands of the acute medicine clinician, allowing key diagnostic questions to be answered rapidly at the bedside and integrated immediately into clinical decision-making.

This intensive one-day course provides a practical introduction to the core POCUS applications relevant to Acute Medicine. Participants will learn the principles of ultrasound image acquisition before developing the skills required to perform focused bedside assessments, recognise common sonographic abnormalities and integrate ultrasound findings into the assessment and management of acutely unwell medical patients. The course also emphasises the appropriate use, strengths and limitations of point-of-care ultrasound in Acute Medicine.

No previous ultrasound experience is required.

Course Format

This course uses a flipped classroom approach.

Participants receive access to the Ultrasound Academy online learning portal several weeks before the course, where they complete approximately 6-8 hours of self-paced learning.

Completion of the online modules ensures that the face-to-face course is devoted almost entirely to supervised practical scanning and clinical case discussion.

Participants practise on modern, high-quality ultrasound systems, with a max instructor-to-candidate ratio of 1:5 ensuring adequate hands-on scanning time, individual feedback and personalised mentoring.

Clinical Applications

By the end of the course participants will be able to:

  • Understand the basic physics of ultrasound relevant to image acquisition
  • Select the appropriate probe and optimise image quality using depth, gain and focal zone
  • Optimise probe handling and manipulation to acquire the desired views
  • Recognise common ultrasound artefacts

  • Acquire the parasternal and subcostal focused cardiac ultrasound windows
  • Assess global left ventricular systolic function
  • Identify right ventricular dilatation and dysfunction
  • Recognise significant pericardial effusion and features suggestive of tamponade
  • Recognise gross chamber and valvular abnormalities relevant to the acutely unwell patient

  • Identify normal lung ultrasound appearances and common artefacts.
  • Recognise sonographic features of pulmonary oedema.
  • Identify pleural effusions and estimate their extent.
  • Recognise features suggestive of lung consolidation.
  • Identify sonographic signs of pneumothorax.

  • Identify and obtain appropriate views of the IVC
  • Measure IVC diameter and assess respiratory variation
  • Interpret IVC findings within the wider clinical and haemodynamic assessment
  • Recognise the limitations of IVC assessment as a marker of volume status

  • Identify the urinary bladder in transverse and longitudinal planes
  • Obtain the measurements required to estimate bladder volume
  • Recognise urinary retention and assess post-void residual volume

  • Perform a systematic focused abdominal examination for free intraperitoneal fluid
  • Identify the principal dependent sites where free fluid accumulates
  • Differentiate free fluid from common mimics and artefacts
  • Integrate FAFF findings into the assessment of the acutely unwell medical patient
